Instructions:

  1. Look over the visualizations and read the textual description carefully.
  2. Go back to the Qualtrics questionnaire but do not close this tab/window with your health score.
  3. Complete all the questions in the Qualtrics survey until you reach the proof-of-work text box.
  4. When filling in the Qualtrics survey, you can revisit this page (health score, visualizations) at any time.
  5. Go back to the tab with the visualizations and text, and copy-paste your proof-of-work code at the bottom of the page.
  6. Go back to the Qualtrics questionnaire, and once you have pasted your proof-of-work code in the text box, complete the Qualtrics questionnaire.
  7. Once your Qualtrics response has been recorded, you can close both tabs.
